2008 Chemical Sector Security Summit Registration Information

Bethesda North Marriott Hotel and Conference Center
July 21–23, 2008

There is no registration fee.  Registration closes when room capacity is reached.

Due to space constraints, each organization and company will be limited to two registrants. Please provide a list of registrants in order of preference for entry. The first two names listed will be given priority registration, while others will be placed on a waiting list.

If your participants wish to be included on the attendee list that will be distributed at the 2008 Summit, please indicate your agreement to be included on the roster in your e-mail. The attendee list will only include your name, title, and organization. The attendee list will not include your e-mail or phone number.

To register, please e-mail the following information for each attendee to chemicalsummitreg@dhs.gov:

1. Last Name
2. First Name
3. Organization
4. Job Title
5. Phone Number
6. E-mail
7. Whether you agree to have your name, organization and title included in the attendee list (yes or no).
8. Whether the attendee is a priority registration or to be placed on the waiting list. 

Your registration is not confirmed until you receive a response e-mail from chemicalsummitreg@dhs.gov including a confirmation number.

Privacy Act - E3 Statement

Authority: The Homeland Security Presidential Directive (HSPD)-7 for Critical Infrastructure Identification, Prioritization, and Protection authorizes the collection of this information.

Purpose: DHS will maintain basic contact information of the Chemical Security Summit 2008 participants in order to allow for the following: communication with participants; admission into the summit; creation of name tags; and creation of a participant directory for contact purposes thereby facilitating participant communication, communication with DHS, networking, and information sharing.

Routine Uses: The information may be shared with other summit participants via an attendee list or directory. Participants must opt-in if they would like to be included in the directory.

Disclosure: Submission of the information is voluntary.  However failure to provide this information may lead to the inability to be registered.
