DHS/FEMA/PIA-001 EP&R Individual Assistance Privacy Impact Assessment

The purpose of this Privacy Impact Assessment (PIA) is to provide details about this collection of information and how privacy may be impacted by permitting victims of a disaster to submit personal information via an Internet-based version of the Federal Emergency Management Agency Form 90-69. The data is the same as the existing paper-based version of the same form, which is covered by an existing approved system of records.
